X-ray IAM (Internal Auditory Meatus) – Townes View is a specialized X-ray analysis that evaluates the Internal Canal (IAM), Petrous Temporal Bones, and surrounding skull base structures. The Townes projection gives a good look at the internal auditory meatus and is useful to view any bony irregularities, fractures, congenital abnormalities, and ear and adjacent skull changes. It is usual to use MRI or CT to get a detailed picture of the contents of the internal auditory cavity, but this X-ray is still useful for initial assessment and some clinical indications.
Purpose of X-RAY IAM – Townes View
Evaluation of the internal auditory canals
Assessment of temporal bone abnormalities
Detection of skull base fractures involving the petrous bone
Investigation of congenital abnormalities of the internal auditory meatus
Evaluation of chronic ear diseases affecting surrounding bone
Assessment of bony erosion due to infections or tumors
Preoperative assessment in selected ENT conditions
Follow-up of previously diagnosed bony abnormalities
Common Reasons for the Test
Hearing loss
Persistent ear pain
Chronic ear infections
Suspected temporal bone fracture
Congenital ear abnormalities
Facial nerve disorders
Skull base trauma
Evaluation before ENT surgery
Structures Evaluated
Internal auditory meatus (IAM)
Petrous temporal bones
Skull base
Temporal bone anatomy
Mastoid region
Surrounding bony structures
Conditions That May Be Detected
Internal auditory canal abnormalities
Temporal bone fractures
Congenital narrowing or widening of the IAM
Chronic mastoid disease with bony changes
Skull base lesions
Bone erosion due to infection
Selected benign or malignant bony lesions
Post-traumatic abnormalities
Benefits
Quick and non-invasive examination
Low radiation exposure
Helps assess bony structures of the temporal bone and skull base
Useful as an initial imaging investigation
Widely available and cost-effective
Supports ENT and neurological evaluation
Risks and Limitations
Radiation Exposure
Uses a small amount of X-ray radiation.
Generally considered safe when medically indicated.
Limitations
Provides limited soft tissue detail.
Cannot adequately assess the auditory nerve or inner ear structures.
CT or MRI may be recommended if more detailed evaluation is required.
Conclusion
X-RAY IAM – Townes View is a specialised X-ray examination looking at the inner ear canals and adjacent areas of the temporal bone. It provides beneficial diagnostic value for evaluating fractures, congenital lesions, chronic ear disease and certain lesions of the skull base. Advanced imaging (CT/MRI) may be necessary to assess soft tissues and nerves in detail for this X-ray to be a good initial test in the right clinical context.
Test information: Fasting NOT needed
Reporting: Within 04 hours*
Usually, no fasting is required unless specifically advised by your doctor.
Wear loose and comfortable clothing. Remove spectacles, hearing aids, hairpins, earrings, necklaces, dentures, or any metallic objects from the head and neck region before the examination.
Inform the radiographer if you are pregnant or suspect pregnancy, as X-rays involve ionising radiation.
Carry all previous X-ray, CT, MRI, audiology reports, and your doctor's or ENT specialist's referral for comparison, if available.
Continue taking your prescribed medications unless instructed otherwise by your doctor.
Inform the radiographer about any recent head injury, ear surgery, chronic ear infection, cochlear implant, or other implanted medical devices.
Remain completely still during image acquisition, as even slight movement may affect image quality.
Arrive at the centre 15–20 minutes before your scheduled appointment.
The patient is positioned appropriately to obtain the Townes projection for visualising the internal auditory meatus and surrounding skull base structures.
The radiographer carefully aligns the head and X-ray beam to obtain the required diagnostic image.
You may be asked to keep your chin tucked slightly and remain completely still while the image is captured.
Additional views may be obtained if requested by the radiologist for a more comprehensive assessment.
The examination is quick, painless, non-invasive, and does not require contrast material.
The procedure usually takes 5–10 minutes.
Reports are generally available within 24 hours.
The film of the X-ray is usually provided within 30 minutes of the scan.
